    1. I understand this is a request form, and submission does not guarantee my event date. All requests are contingent on availability of the master calendar. I understand confirmation will be given after my date has been secured. By submitting this form I confirm that I have read, understand, and agree to abide by the Facility Use Policy of Camas Church of the Nazarene (CCN), and submission of this form shall act as my authorizing signature of said policy. I understand that I assume all risks for loss, liability, injury, cost, or expense that may arise during, or be caused in any way by my use or occupancy of CCN facilities. I further agree to hold CCN and/or its employees and representatives free and harmless from any loss, claims and liability, or damages, and/or injuries to persons or property that in any way may occur as a result of my use or occupancy. By submitting this form, I hereby certify that I will be personally responsible for any damages. My submission certifies that all information on this application is true. I understand and agree that any misstatement or omission of material fact herein may cause forfeiture of my deposits.
    2. I understand that authorized personnel and trained technicians shall be paid at a rate of $20 per hour per person. Further, to offset the overhead costs of building us (power, water, heating/cooling, restroom supplies, custodial, etc). I understand every event requires a non-refundable $50 deposit before my event can be confirmed on the master calendar. Fees can be paid here.
